Mine was, in that it gave first-request approval, paid $10,000 or so to the hospital and another $6,000 or so to the surgeons, without blinking an eye. But what did they decline on the grounds it was " major medical " ? The $38 rental fee for the suction machine my surgeon insisted I take home. (which I never used). Go figure!
