Take Care Vera Treatment of Erectile Dysfunction in PD Treatment of erectile dysfunction with sildenafil (Viagra) in parkinsonism due to Parkinson's disease or multiple system atrophy with observations on orthostatic hypertension IF Hussain (lead author) J Neurol Neurosurg Psychiatry 2001;71:371-374 Sildenafil is safe and effective for PD patients, but may cause severe orthostatic hypotension (drop in blood pressure upon standing)in MSA patients, according to this report. 12 PD patients with erectile dysfunction received either placebo or 50 mg sildenafil for 10 weeks, with dose adjusted as needed, followed by crossover to the other treatment arm. Two patients did not complete the study. In the 10 who did, 9 titrated up to 100 mg, and 1 down to 25 mg; Significant improvements were seen in ability to obtain and maintain erection and there was a marked improvement in sex-related quality of life. Post-dose blood pressure determined while lying, sitting, and standing was not significantly affected in PD patients. 6 MSA patients were randomized before treatment was stopped due to severe blood pressure drop (approximately 50% systolic, 35% diastolic) in 3 patients 1 hour after the first treatment (which was administered in the physician's office).
