Hi,

My first consult with Dr. Vanguri is May 25th. I've

hunted all over the house and CAN'T find my

paperwork!!!! I had a car accident the end of April,

I think the paperwork got lost in the shuffle. I

remember reading that if you don't have your paperwork

with you for the day of the consult, don't

bother...they will need to reschedule. Has anyone

gone to a consult and been turned away because you

didn't have the paperwork completed?

I'm on leave from work (4th week now : ( ), otherwise

I'd try to get the paperwork faxed to me there and

complete it in a hurry. I'm afraid it's to late for

snail mail. I will call Dr. Vanguri's office tomorrow

and see what they recommend, but I just thought

someone here could shed some light on the topic and

give me hope?!
