This way if Jeff was unable to communicate, or his parents decide to fight his wishes, I am authorized to carry them out. It's a terrible burden to have, but he was afraid his mom would want to keep him alive at all costs, and he does not want that in the end. That is the hard part when you have to really admit these things are happening. And there is no way out. Keep your chin up.
