"They told me too, that BC/BS was paying for the DS and then they started to disallow it. They are not sure why"

My insurance company BCBS PPO just paid for my surgery. I had NO copay at all. Here is what was billed and what they paid under the contract.

NYUMC billed $24,824.73

BCBS paid on 4/24 7,673.34

My balance 0.00

Anesthesia 4,180.00

BCBS paid on 4/24 1,686.00

My balance 0.00

I bet the doctor is not a contracted BCBS doctor. Go to http://www.bcbs.com/

and browse the site. You will find a link for your state and from there you can find out what doctors and hospitals have a contract with BCBS. If you have a little suitcase on your insurance card you can go ANYWHERE your primary care physician refers you that accepts and has a contract with BCBS.
