Did you gain weight on purpose to hit the 40 BMI? I think what you should do is look on Obesityhelp.com. They have some sample appeal letters in the before and after photo journals...and there may be a section with it set aside as well. I'm not involved in this stuff, and my appeal wasn't written by me. I just went and had a sleep study (where it showed that I had sleep apnea), and my referring physician sent some kind of supplemental information to them. Do you have co-morbidities? Are they documented? I would just write a letter telling them about the co-morbidities...and tell them that you gained four lbs. in a certain amount of time because____. I don't know why you gained weight, if it was just normal, or it you were actually trying to get to a higher BMI? But for me, gaining four lbs. happens everytime I'm on my period...it's a normal weight fluctuation. So...just explain all of that (if it's truthful), and go from there. I can't really give any more advice, because it is a conflict because of my job. Good luck!
