Guest guest Posted June 5, 2001 Report Share Posted June 5, 2001 Ok..I've worked hard the past few days and think I'm about to pull it all together, make a copy for myself and mail in my appeal. I have a letter from my employer...It states they are in support and hope the exclusion is ignored and I am qualified for medical necessity. I have a letter from my therapist stating I am a good candidate and that I will be able to move on and focus on the smaller issues I have and probably not need as much therapy or anti depressants since so much can be traced back to the weight issue. I have a letter from my new PCP saying I am severely obese and called it life threatening. Very powerful and almost hard for me to read...but I need it. The surgeon stuff is in already and I think from talking to them I don't have to resubmit, just reference it. I have pictures of myself that I scanned from over the years where you can see my weight changing. I am picking up my med file tomorrow from my old dr (she called me..oh I sent that in..but i dont think she did...the original letter was mailed to me.) to show my weight over the last 5 years...since that is a condition I must meet. I have my diet list about done showing the attempts I've made. I don't have anything saved so its rather made up...but I tried to stick to somewhat the reality. I've never had alot of success in dieting...and when I did..it obviously wasn't long term or I'd not be almost 400 lbs! So...what else? Oh I did include some stuff on the surgery and I wrote a letter useing the one from obesityhelp but made it much more personal and directly towards myself. Ended up about 5 pages. Am I forgetting anything? Would a letter from family member help? What else? I think from what I've been told it can be deemed medically necessary and I meet the criteria so I have a good chance but I want them to understand I'm not playing a game...I am going to do this. Had my consult the first week of Feb. with Dr. Booth. So, Honey, I know where your at!! GOOD LUCK!! Just an idea, my mom kept journals (she passed away) and I am going to even check those to see if she made references to my weight and the " programs " I tried. If the information is there, I'm gonna make copies and send with my paperwork!! Keep us informed!
