Brand New with lots of questions

Hi - My name is Melodie in Colusa, CA. BMI =43. I

have just made a consultation appointment with Dr.

Rabkin in SF for July 30th. As this is a 2 1/2 hr.

drive for me can somebody give me some ideas on what

to have with me when I go to see him. Do I take

bloodwork with me or does he do it on the day I am

there. Should I get some letters referring to medical

conditions from my doctors, i.e. allergist (sleep

apnea), PPO, Psychologist, Podiatrist, Chiropractor???

I want my insurance to accept me the first time. I

have HealthNet. Does Rabkin's office do the request

to the insurance or should I?

Sorry if this is dumb stuff but I am worried, excited,

expectant. Thanks for any info.
