Does anyone have an outline topics covered in Dr. Drooker's psych letter?

Hi,

I am seeing a phd psychologist who am going to ask to write me a

letter to submit to my insurance company. I have decided not to use

the psychiatrist Dr. Gagner uses, Dr. Drooker, because of the

negative things everyone has said about him and because he does not

participate in my insurance plan. I would be interested in learning

what topics are contained in his letter so I can ask my psychologist

to cover all the right points. I don't want to know anything

personal about anyone, just an outline of the topics that should be

covered in order to get insurance approval.
