Choosing a Surgeon

Hi all:

Janice motivated me to complete a project I've been working on for a

while. What I did was to take my notes on how I chose my surgeon and

turned them into a sort of fill-in-the-blank form. It's in both Word

and Text formats, and I uploaded it to a folder called " A Guide to

Choosing a Surgeon. "

Feel free to use it, change it, modify it, give it to friends, or

print it out and blow your nose on it if that's what spins your

beanie. (:

I hope at least somebody finds it helpful.
