letter of appeal?

I had asked for sample letters once before. I had got denied from my

insurance for the Bilo/DS and I now am constructing a letter to mail to them

explaining the differences between any other procedure and the DS. Just to

make sure that they understand, and I had a letter from a lady I believe her

name was , it was a wonderful letter, and she had said that I could

use it for info. to contruct my own. Well......I lost it! I cannot find it

for the life of me, I know I kept it in my computer somewhere, but I am

unable to locate it, so I believe I must have deleted it unknowingly. So if

you are out there .....(if I am right with the name) could you send it

my way one more time? I also would like anyone elses help as well. I am

printing out letters and highlighting the points in each one that I want to

include in my letter and then I will be constructing my own. The one I am

missing had several references in it and then listed at the bottom of the

letter in numeric order.
