They do a fair job of distinguishing the DS procedure from the RNY and point out why the DS is better for me. I used a letter that Magruder (I think) posted at one point to construct a letter to my PCP explaining what I needed him to write. He, in turn, used much of my letter in his letter. They are all in the files section under the Insurance Appeals folder. Let me know if you cannot access it and I can email copies if you are interested. There are no direct references listed in my letters, that I can remember, so if they don't meet your needs, I wish you luck in finding . Hopefully you will be successful in your appeal like I was.
