Guest guest Posted May 12, 2004 Report Share Posted May 12, 2004 , I was on it before, and I know that you have to take your proof of residence, ID, and proof of income to your appointment. They ask you about your diet, and if you have any weight issues, and refer you to a dietician that should be on staff if you need it.
