My son Max is 10 weeks old and has bilateral cf. He is now wearing the dbb. They are set at 45 degrees on both feet. I saw on another message that there are two different types of bars the and Markell. What is the difference? How can you tell what kind you have? Another concern/question I have is that at Max's last appt. he got the dbb on for the first time. The doctor had the nurse put the shoes on. The doctor wasn't in the room and the nurse couldn't get the shoes attached to the bar. The doctor then came in, looked at the shoes and didn't say much. He then sent me to Orthopaedic Services (where we got the dbb) and they put them on correctly and showed me how to use them. I just felt that his doctor kind of pushed it off and didn't make me feel very confident in him. Am I being paranoid? Is this how it works? Thanks for listening. Guest guest Posted March 7, 2005 Report Share Posted March 7, 2005 I'm not sure how things work at your clinic, but at Gillette in St. , they have an entirely different department that takes care of the DDB and the doctor doesn't help with the shoe/bar fitting. This didn't bother me too much since my son really hasn't seen his doctor all that much. He's mostly dealt with the cast techs up until this past week. His doctor stops in briefly at every appointment to check progress, but most of the " manual labor " was taken care of by others. As for the shoes, I wondered the same thing after reading the posting about the different types of shoes. If you look at the shoe box (which hopefully they gave you!), it should have the name of the shoes on it. How is your son handling the DDB? My son, Aleksander, is having problems with it. We have to go back with him sometime today to get things adjusted, or perhaps to have casts put back on. Guest guest Posted March 7, 2005 Report Share Posted March 7, 2005 Hi , Welcome to the group. Normally feet are corrected to a 70 degree angle and that is was the shoes are set at. If children have a unilateral clubfoot then the noncf is set at 45 and the cf at 70. When we received our shoes and bar we had a prescription and went over to the orthotist who took it from there. Our dr. did not get involved in the shoes/bar. The shoes look like Birkenstock sandals, the Markell shoes look like little white boots with open toes. Did you dr. mention what method he used to correct Max's feet? Look forward to hearing more about your treatment. Guest guest Posted March 7, 2005 Report Share Posted March 7, 2005 , Welcome to the group. I am concerned your son's shoes are set at 45 degrees. You mentioned your son is bilateral, so both feet should be set at 70 degrees. If you read posts from yesterday and Saturday, there was a big discussion about the DBB and degrees that should be set for each situation. I strongly recommend reading them. If your doctor prescribed the 45 degrees, then he/she is not following the Ponseti method correctly. So then yes, I would be paranoid. It seems your doctor doesn't even care. Who is your doctor? Where are you located? It is definitely not too late to get a second opinion. I know this isn't what you were hoping to hear, but there is a huge red flag with this doctor, so I would get another opinion ASAP before major relapsing occurrs. Please let us know what you decide. Guest guest Posted March 7, 2005 Report Share Posted March 7, 2005 Welcome, & Max- I concur with the other posters that it sounds like Max's doctor is modifying the bracing protocol- if he/she is even using the Ponseti method at all. The last casts must have been set at an outward rotation of 70°- otherwise setting the shoes at 70° won't be comfortable for Max. The foot would have appeared almost backwards (pointing out toward the hip) in the last cast if it was at 70°. If you go into the files section here on the group's website and click on the bracing folder, there is a folder inside with information from Hall at APO in Iowa City (orthotics group) with how the brace should be set up, what components to order etc. If you print out that information it might help! This information is for the standard brace, the adjustable length red bar with the Markell Tarso, open toed shoes. Most babies use this brace. The shoes are custom made and are *mostly* used on children with a-typical clubfoot for whom correction is difficult to maintain. I don't think you're being paranoid at all. You have a God-given gift called " Mommy instincts " and I think they're telling you that you're not getting the proper information and support from your doctor and staff. Even if the doctor isn't the one to fit the brace- they should have given you information on what to expect and answered your questions about it. If you would like to tell us where you're located, or what doctor you're seeing, perhaps we can suggest some other doctors to seek out as a second opinion- or other families in your area to compare notes with.
