methods to keep DBB on?

My son was born with bilateral club feet. He was treated with the

Ponsetti method until Febuary when we had surgery. He is now in the

DBB. I was wondering if anyone had any suggestions to keep the bar

on. My son seems to always manage to get them off no matter what I

have tried. He will be a year old in April and I would like him to

get past this part of his treatment successfully. Thank you for any

suggestions you might have.

I haven't seen any responses to this, so I bet you all missed it too. I

have sent her a copy of the tips doc. But I wanted to respond and bump the

mail down so people will see it.

Which surgery are you talking about Aliza? Can you tell us more about his

treatment so far? Who is his doctor?

OK - y'all can help her from here on out... I'm off for a 4 day vacation

with the family, I will of course have some internet access. But I'm outa

here for now! More packing to do... too bad I just ate some lunch and

feel like a nap!

I feel like an idiot for saying this but I am not to sure about the name of the

actual procedure. So far he has been treated with the Ponsetti method of serial

casting from 2 days old until 4 months when he had achilles tendon lengthing.

He was then placed in DBB but was not fitted properly and not actually ready for

them. We are a military family overseas thus we don't have a whole lot of

options when it comes to doctors. I believe that more could have been done

sooner. We have been sent to other military hospitals so they are doing all

they can for now which I truly appreciate. Sometimes I do wish I had a civilian

doctor but they have done the best they can with what they have. I just need to

share with other people who are going through this. Just to learn more about it

and not feel so alone. I believe that the best thing I can do for my son.

Hello Alizia.

Are you saying your son had another surgery in addition to the

tenotomy(achilles lengthening)? 4 months is a little long for serial

castings. Can I assume the casts were changed weekly? Did they

manipulate the foot before each cast was reapplied? The usual Ponseti

protocol is only 6 weeks of casting (or less), tenotomy procedure, then a

3-week cast, then the DBB 23/7 for 3 months. The DBB wear time reduces

slowly after the 23/7 to 16-18 hours, then eventually 12-14 hours when

they start walking. If you go into the files section of this list, there

is a DBB tips and tricks list. There are so many tips to help keep the

shoes on. Please also keep in mind that sometimes if the shoes don't stay

on, it can mean the feet aren't fully corrected. The best thing to do is

read through the tips list and try all of them! If the shoes still aren't

staying on, then there may be a correction issue.

You have come to the right place for help and support. Hang in there.

Where are you exactly?

Alizia,

Since you have limited choices with doctors, I would suggest that

you print off copies of the medical booklet at this site:

http://www.global-help.org/publications/ponseti-cf.html

Which explains the Ponseti method in great detail. Then you can

have your son's doctor(s) review it and go over it with you to see

if there is anything that they can learn from it and apply to your

son's treatment. Please note that there are some graphic medical

pictures in the booklet.

What country are you in? Have you looked at the listing of the

doctors on Dr. Ponseti's website to see if there's one near you?

Let us know if you have any specific questions or need any

additional information- and keep us posted on how things go!

Hopefully the FAB tips document will help, but as was stated before,

if his feet aren't fully corrected, then you may have issues keeping

the brace on regardless of what you try!

Regards,

Yes my son has had two seperate procedures. He was casted weekly for the first

4 months which they told me was common. I lot of the information I have

received is different from what I have told is the norm. I am currently in

Japan so I am pretty limited when it comes to a second opion.

Alizia,

There is a Japanese doctor listed on Dr. Ponseti's website- I don't

have any first hand knowledge of this doctor, and I know it may not

help you since you're in the military medical system, but here's the

information:

Natsuo Yasui, M.D.

Department of Orthopaedic Surgery

School of Medicine

University of Tokushima 770-8503

3-18-15 Kuramoto-cha

Todushima 770-8503 Japan

Tel: 81 88 633-7238

There is also an email address for the doctor at Dr. P's site.

One of the families on our board was living in Japan also when their

son started treatment. The mom's name is Jen and the son's name is

Calvin. You can read about their story if you search the archives

for " Calvin " (you'll have to scroll through the historical messages

small blocks at a time by hitting next using the " search archive "

function).

I know that Jen brought Calvin to Iowa from Japan when he was around

1 year old (I don't remember exactly how old) after unsuccessful

treatment there. They now live in the States.

Hope this helps some-

It doesn't sound like a real Ponseti method to me. Four months is way too long

to be casted, and if the feet were corrected the DBB would stay on and be OK.
