Guest guest Posted March 27, 2005 Report Share Posted March 27, 2005 Hi - You first need to call the members number on the bottom front or back of your healthcare card and request a claim form and instructions on how to fill it out. Fill out the claim form and submit it to the address tbey give you. They will deny it and then you will need to file an appeal. When they deny the claim they give you an address to send a letter of appeal. You then need to type a letter (we can help you with that) explaining the importance of the Shoe and Brace. You then will get a letter accepting your appeal and a check. It is about a 3 month process to payment. It is a hassle but worth it. He should include the codes needed for the shoes and bar on it. If not the codes we used were Shoes-L3252, Bar-L3150. Then submit both forms to the insurance company. Make sure you make a copy for yourself. You might have a deductible to meet before you get reimburst. I recommend you submit it anyway. I haven't had to fill out forms for anything either, but we've only gotten the bill for the Markell's so far. I'm looking into what I have to do for the s. I contacted , and they indicated that insurance hasn't approved their shoes yet, so there may be a different process for getting them paid for. I would call your insurance company and find out what they say. They'll likely tell you to get the code for the shoes, and who will be billing them for the shoes. Guest guest Posted March 28, 2005 Report Share Posted March 28, 2005 Hi , We have battled a few insurance companies to pay for Collin's treatment at the hospital we choose and for items such as straight last boots for day time, DBB/FAB, etc. Over the last few years what has helped the most is for us to request only 1 person to deal with. Collin has a case manager over at Aetna. It helps because they get to learn about the needs without having to keep on stating your case. Also even when my husband switched jobs, and changed types of coverage, we choose Aetna again just a different type and his case manager stayed with us (I had to request this and beg a little). I remember the 1st year of Collin's life was the hardest battling with them for a qualified doctor, etc. It was really emotionally draining, but it really does get easier over time. A few times I would have the doctors office call them and tell them that what he needs is a medical neccessity, etc. If I can help you in any way please feel free to email me at chele323232@... I have not dealt with the shoes, but all we needed was to pick up our shoes after having a prescription for them. The ortho. office sent in the necessary paperwork. We didn't have to do anything for the Markells, and we only pay 10%. Has anyone's insurance done this, and knows if it is just a easy for the shoes? The foot impression kit was sent overnight to and our Dr. (Scher in NYC) sent Blue Cross two letters. One was a letter of medical necessity and the other was a longer and more detailed letter exlaining the Ponseti method and how the shoes/bar is a vital part of treatment. That is why you also need the letter of medical necessity. I believe he is in the process of trying to. When we first started out we had the Markells and the insurance only covered the bar. That's what I have, and I'm trying to battle with them right now because they say they won't cover the shoes & bar. They say it has to be " an integral part of a leg brace or she has to be a diabetic " . So did you get them to cover Collin's bar & shoes? (Or at least a portion? If your wearing the s perhaps you could check with and see if he would give you one with the information on it and then just have your treating Dr. sign it.
