Guest guest Posted August 8, 2005 Report Share Posted August 8, 2005 Greetings, Some time ago I heard a presentation at a farm worker health conference in North Carolina that described how community health centers in the east coast were dealing with the health records of migrant farm workers who went back and forth between two or three states. I believe the workers carried their records with them from place to place in a 'card' or other portable format. Would anyone be able to point me in the right direction? I'd like to explore that model and possibly apply it to non-migrant people who have been exposed to environmental hazards and need to document it in their 'permanent' health records but - because changes in health plans affect continuity of care - do not have a steady primary physician.
