Guest guest Posted May 6, 2001 Report Share Posted May 6, 2001 On the immunization form, there is a space for religous exemption which you can just sign, and that's that!!, and a space for medical exemption, which has to be signed by a Dr. with reason given--much more of a hassle than religious exemption.
