Re: Canadian parents... and surgery after tenotomy?

I have NO idea on this... and would default to Dr. Ponseti (poor, poor Dr.

Ponseti...) on this one. Ask for copies of the u/s where it shows these

bones that need to be surgically placed and send them to him asking about

it. I'm inclined to think that the bones can be manipulated into place by

hand because that's the whole basis of the Ponseti Method, but I'm no

expert by any means. Since you can.... check it out with Dr. P and then

you'll know.

Dayle I don't want to get excited here because I think we need more information

but the first question that comes to my mind is this: (maybe not a question

but more of a comment?) um... if he's been casted and has had his tenotomy -

theoretically his feet should be totally corrected now, awaiting only the

tenotomy to heal and begin using the DBB.

Can you tell us how many casts your son wore? Were they full length (toe to

groin)?

I'm in Montreal and my son is being treated at St. JUstine's by Drs

Grimard and Morin. Just wondering if anyone has any experience with

them. They are using the Ponseti method.

Dr. Grimard suggested he may need a further operation to place the

bones in the foot. That need would be re-evaluated in a few months,

as the improved flexibility might allow them to move into proper

position.

clubbed feet, as did his father. I guess I'm just getting nervous

based on some comments someone made. Just looking for some comments.

A while back there was a parent posting here (forget who) - they were facing

surgery because their dr. said the baby's foot bones were out of alignment...

anyone remember this? Club foot is where the bones are out of alignment, with

proper casting they are put back in place non surgically and if I remember

right, this parent found a Ponseti dr. who did that.

Hi Tia,

Hopefully will see this post, she and her hubby Dan live in

Montreal and see a Ponseti trained doctor and have had a great

outcome with their daughters foot. Personally, the mention of surgery

would raise major red flags with me!

I like you would be searching out as much info as I could regarding

these doctors, like are they appling the Ponseti method correctly? If

so, even a difficult case should be able to be treated by casts!! Our

daughter was brought to full correction with 4 casts and no tenotomy,

we see a very skilled Ponseti doctor in Vancouver BC. I would soooooo

get a second opinion even if my doctor was mentioning the

*possibility* of surgery!

It is good that you are here! You will learn so much from the people

on this site! I never even knew what club foot was before my daughter

was born with it. I feel very blessed to have found such a good

Ponseti doctor and this site!

Hi,

The casts were full length, and he had a total of 6 casts prior to

the surgery. It was going to be the week the 6th was put on, but

there was an emergency surgery that bumped us that day. So the last

cast was done pretty much wihtout any manipulation.

I am seeing the doctor on tuesday, the post-op cast will be changed

(on for 2 weeks) or removed. Do you have any questions to suggest I

ask?

I'm still not sure - anyone else want to chime in here? I guess about all I

can speculate is this: if your son wears his DBB with out any issue, you can

generally figure his feet were corrected properly. If he has big issues with

the DBB (pain, blisters, pressure sores, heels not fitting, etc.) then they

probably are not corrected properly. The DBB won't do a thing for correction,

not sure if you know that or not - they maintain correction but don't give

correction. Anyway - if in a few months your dr. is still talking surgery I'd

definately seek out a 2nd or 3rd opinion first. Once an operation takes place

there's no going back.

Hi Dayle-

I would definately get a second opionion from a certified Ponseti

doctor with lots of experience. There are many doctors practicing

the Ponseti Method, but not ALL of them are able to manipulate the

foot enough to avoid surgery. Since your doctor mentioned possible

surgery that would suggest he can't manipulate to the level your son

needs. With Ponseti method everything is corrected by the tenonomy

time and that is the last thing. You don't do the tenonmy and still

have bones out of place. You are suppose to have the whole foot

corrected and then just do the tenonomy to bring the heel down.

Sorry I'm in the US, so don't have some to recommend. Did you check

the Ponseti Certified list???

Hi Dayle,

We're in Mtl also!!! please email us off line tec8f20@... and we'll

give you our phone number if you want to talk with us- we went through this

5 years ago with our daughter Emma (our story is at

http://www.vh.org/pediatric/patient/orthopaedics/clubfeet/emma.html ).

Thanks to Emma's great results (by Dr. Ponseti) the Shriner's and the

doctors from other hospitals working out of the Shriners gradually switched

from surgery to the Ponseti method. So this means that they have only been

at it a few years and they are not certified in the method yet. Basically I

can't say how well they are doing.

If they are warning you of a surgery I would get a second opinion from

either Dr. Ponseti himself (the best in the world) or if you want to stay in

Canada either with Dr. Tim Carey in London, Dr. Pirani in Vancouver, or Dr.

son in Kingston.

Most importantly if he's going into the FAB the feet should already be fully

corrected. The FAB only maintains corrected feet. Is irritable in

the brace now? How are your husband's feet doing today-- he must have had

surgery?

The name Morin rings a bell. Dr. P and Morcuende (in Iowa) mentioned that

he went for training in Iowa a few years ago. I don't know how well he's

doing the method.
