Re: Ponseti Brace instead of Markell

By the *Ponseti Brace* you are referring to the shoes - thought

I'd clear that up to a few newbies who may not know the difference. But

otherwise I totally agree with you, they are great shoes although many many

babies do just as well with the traditional Markell shoe.

I wanted to add also that it's almost funny in hindsight as we enter in to the

cf world how dark and dreery the door way appears but once in it, it's hardly

dark at all and we all end up joking and enjoying our journey, as strange as

that is to say. The funny things these kids can do wearing that dang DBB or

casts is almost a hoot. Learning to change a diaper WITH OUT the bar on....now

there's a challenge! haha ...the stupid things strangers say....there are funny

things along the way that lighten the load. And there is this group which

lightenes it even further as we share both the good and the bad here.

The journery through correction is not nearly as heavy as we anticipate, it just

seems that way when we learn we'll have to travel that road. But pack light,

step with confidence, enjoy and look for the bright spots that are there because

they will light your way.

I just wanted to post this because when I first had my ultrasound and they found

Jonathon's club feet I read a lot of posts on this bulletin board, and although

I hardly wrote a thing I learned a lot. It really helped me to hear what club

feet were really like, and now that I have the experience of having a kid who

made it through correction pretty easily I figured I should contribute. I was

hoping that for some of you struggling with the Markell shoes this might be a

help!!!

Jonathon has used the Ponseti brace instead of the Markell shoe, and has had

excellent results. His feet are showing an extremely high degree of retainment

of the correction. Jonathon has never had a pressure sore, blister, raw spot,

or any acute issue from the brace. He did dislike getting it put on in the

beginning for the first week or two after the casts were removed most likely due

to increased sensitivity after treatment. It did take us a couple weeks to get

his heel as " down and back " in the shoe as the instruction picture showed, but

we worked our way into it, and his feet are doing very well!

We have paid cash for the brace as Mr. is not quite all set up as a

provider with the insurance companies yet. The initial brace is roughly $350

with $22 of shipping and handling. The bar does not need to be replaced and is

$70 dollars or so. We ordered the shoes right on Mr. 's website after

discussing personally with him.

We contacted Mr. (, http://www.mdorthopaedics.org/) prior

to Jonathon's heel cord lengthening (tenotomy). The day of the tenotomy, after

the cast was removed but prior to the surgery, we had Jonathon " step " on foam in

a box (Mr. can send you the foam in a box or your Dr. may already have

some if they work with Mr. .) We Fedex'd the box to Mr. who

was expecting them. He had the shoes delivered to our home before the removal

of the casts. We brought the shoes with us to Dr. the day the last cast

was removed, and he put them on immediately after the removal of the cast.

I would strongly suggest people find a way to get their children these new shoes

instead of the Markell shoes. They are so wonderful! If your Doctor hesitates

to use them, give him or her the website information. If they are really

Ponseti certified they should be supportive. The brace was designed with input

from Dr. Ponseti. They can also call Dr. if they would like a peer

wrong with an infant, but it is an already fading memory. Jonathon at 5 months

old has " normal " looking and functioning feet. He exhibits no pain when we

point or flex his foot and he is a happy go lucky little baby. I feel so

relieved that the worst of our ordeal is over and wish this for every family and

child who was born with club feet.

I'm curious, but I thought the shoes (or Ponseti Brace, we're so

bad here. We've called them shoes this whole time and has

named it the Ponseti Brace from day one... we really should start using

that name instead of . Great testament to but I think it's

better to call it the Ponseti Brace - it says Ponseti right on the bar!)

for the regular CFeet was to be worn at first and then the child would be

transitioned to the Markell's. What's the plan for Everett anyway? And,.

in all seriousness... the Markell's are good shoes. I think it's great

that the 's are out there, they are needed but the Markell's do

work fine for most babies with fully corrected feet... AND parents who are

given appropriate information about applying them. Getting that

information out is a huge problem though.

With all that said... I want 's (Ponseti's lol!) for Darbi. I

think with their flat sole that she might be able to walk in them unaided

and they don't look like they'd tear up my floor without padding (which

inhibits her walking unaided because it's so thick). Plus the toe area is

much more open and she's got that curvy toe that I want to go back

straight. I think these shoes would do the trick with that.

I've often wondered that too Kori. I was under the impression that these shoes

were only made in infant sizes and that the kids would be transitioned to the

Markells eventually anyway.

My other question would be is it really feasible for anyone to be telling ALL

new parents to insist on the s instead of the Markells? What I mean by

that is that is only one man. Currently he is making all these

shoes by hand. If he is going to keep doing that and all parents start

insisting on them, there will shortly come a time when it will be physically

impossible for the supply to meet the demand. Some kids who would have done

quite well with the Markells will be wearing s while some kids who

really need the MItchells due to atypical clubfeet won't be able to get them.

I've said it before, and I'll say it again....the Markells have worked just fine

with little to no problem for thousands of babies. When used correctly on a

competely corrected non-atypical clubfoot there shouldn't be a need for the

s.

The s definitely do have a place in treating the atypical clubfoot and

it is a blessing that they are available to these kids who had no other options

before.

Again, I think the focus needs to be on educating doctors to correct feet the

right way and to educate parents on the correct usage. Until the supply

problems can be worked out this seems to be the better choice.

kids certainly are amazing in this DBB. This morning,

Kelsey learned that she can climb over the back of the

couch with the brace on. SHe can climb all of the

stairs to the 2nd floor. I am so glad we took her out

of her crib or she would be scaling that I am sure.

Hmm, the plan for Everett?????? What plan? You mean I'm supposed to have a

plan? why didn't anyone tell me! Yikes! hahahah

I started him in the Markell shoes when he was about 7 weeks old and he had

severe issues with them causing pressure sores on his heels. We thought the

actual shoe was faulty and tried a new pair (Markells) with same results; that

when Dr. Ponseti said to call MItchell, *They fit like a glove!* he told

us, so we called and he built us a pair back when they were just

prototypes. They did the trick! So we went with a 2nd pair of Ponseti Brace

('s) that we're in now. I'm seriously considering a regular pair of

Markell's next though, because of the price difference. I hope his feet are big

enough now they will fit him with out issue and I understand some improvements

have been made to those shoes too.

His basic plan is to just wear the DBB till Dr. Ponseti tells us he doesn't

have to anymore. He's 17 months old tomorrow, wearing the DBB 14 hours a day.

Just curious, what size shoes does Everrett have?? is still in a two and

they are still

big on him! But the puffiness on the top of his feet is actually going down a

bit and the

creases on soles are getting better! Yay!!!!! :)

They say 2 on the back of them, so I assume that's size 2. These are just now

fitting him like a perfect fit, so he's on the way of growing out of them but

I'm sure that won't be for several more months.

I'm thrilled to hear is making so much progress! Walking, putting his

heel down - and you removed those wedges??? Wow, that was fast!

Yesterday at Grandma's Everett was showing off to her all his walking styles.

He can walk straight legged waddling like a duck, really leaning to the left

then to the right; he can walk backwards. He can run (kind of); then stop

suddenly and turn. He can spin circles until he's dizzy....he went on and on

showing off various walking techniques, it was funny.

My doc went to see Dr. P back in November, when we were going

through deciding whether to put Ava in the DBB like Dr. P says or

the DAFO, which is what my doc prescribed. Anyway, when I talked to

him more he said that when he was in Iowa with Dr. P he wasn't even

really using the Markells anymore (not sure how true that is) but is

mostly prescribing the Ponseti Brace/s for everyone. He was

going to put my daughter into the s, but we got a pair of

Markells from someone on here & decided to give them a shot first.

Thankfully they've worked fine (saved us some $$$!) but just thought

I'd let you know what our doc said. At least that's the impression

he got from Dr. P.

the plan, I meant to use 's forever or to switch to the Markell's

eventually. I think you'll find they do work well, especially on a nicely

corrected older foot. I don't think they could cause a sore on Darbi's

foot now if I tried (loosened them, etc.)

I didn't know you had sores too, no maybe I did... I can hardly remember

my own name recently... Do you think it was because you weren't getting

the heel down? Because we KNOW Dr. Ponseti had his feet fully

corrected. Oh wait... I do remember something about you saying you

thought it was user error... it's coming back to me now... I stand by

the idea that these shoes work, and can work well on a newly corrected tiny

little foot. But really, only if you get the feet in them properly. Which

is not easy if you don't know how to do it, heck, it's not exactly easy

when you do know how to do it. I think it's a combination of not pushing

the heel down and not getting them tight enough (as a result of not pushing

the heel down enough) that is the cause of the problems we see with fully

corrected feet and not the shoe's fault. We're told to make them

tight. But if the heel is rising up, you can tighten them with vice grips

and it's never going to be tight enough.

You know its interesting what you said about Dr.P not using the

markells anymore. and i say this because when we went to see him in

december my daughter was wearing the markells. he asked me to take

them off and told me those shoes would not help my daughter, he said

she needed the mitchell shoes. i agreed and they have been great on

her. i dont mean to sound negative about the markells although my

daughter could not stand being in them and got sores from them but

it was because her feet were not corrected. but i wonder why Dr.P

sounded so negative about them if there are a lot of children out

there wearing the markells. Dr. P told me he did not want her in the

markells again, just the mitchells and they have been great on her.

just thought i would put my 2 cents in and see if anyone knows why

Dr.P pushes or favors the mitchell brace(ponseti brace).

I think you raised a good point. I am curious if it's because the

atypical cf wouldn't do well in the markells maybe Dr. P asked them

to improve their shoe to accomadate the atypical foot and they figure

they help x-amount of feet the shoes are fine. So then came out

with the mitchells which would fit any type of cf so he pushes for

them. I don't know the real answer its JMO.

Our problems with the Markell boots.......I just don't know. On that first

pair there really did seem to be a problem with the back seam, sort of a lump

sewn in to it and we really thought that was the culprit...but the 2nd pair did

the same damage to his foot. I do remember how huge-ly difficult it seemed to

get them on him - I mean it was awful trying to get those damn things on. I

absolutely hated trying to deal with all that string and crap, trying to hold

him still, not being sure if the heel was in place or not. We took all the

string and tongue out, placed his foot in right, drew the little line where his

toe should go, put it all back together, put his foot in, the toe seemed right

at the line and stuff, but he cried non stop and got sores. I never noticed the

foot slipping either in or out (based on that line). They just hurt and made

sores. I'm not really sure why.

The /Ponseti sandles solved all of that. He fussed a little bit when I

put them on at first, but it was just fussy, not screaming in pain, and soon he

didn't even do that anymore. You SEE the heel, and there's just three little

buckles like you'd have on any old pair of baby sandles. They take all the

guess work out of it so I can see why a lot of parents would opt for those shoes

in the beginning. If the 's weren't so expensive I'd stick with them.

But I know now I can cut the back out of the Markells, which I didn't know then.

I'm guessing all told it ws a combination user error and his feet being so small

too.

It only makes sense to me...the Markells have been around forever

(the basic concept, I know they adjust the style occasionally) and

are not specifically made for CF, they're used for a variety of

things, right? So the s or Ponseti Brace is designed by Dr.

P for his CF babies. It's like the difference between a shoe you buy

at Payless for 11.99, which gets the job done, but just doesn't

compare to the custom made Italian leather jobbers for tons more

$$$, but are specifically made for your foot. Obviously one is

better than the other. Now for me, I'm Payless all the way! But I

would have gladly handed over my measly savings account for a pair

of s if my daughter needed them.

I think he's in the process of opening a production plant.

Also, my perspective was if there was something that was better I

wanted it for my son. I'm not the type that will mess around with

less than the best when it comes to my kids health.

With the size thing, he sent us our second pair, and they are big

enough to carry him through at least 18 months so I'm just going to

play it by ear and hope he keeps making them for us until we don't

need them any more.

When we received the Markell's for our daughter, we didn't know there

was any other shoe available for cf treatment. Besides, it was never

an issue for us as she did well in them, no problems at all. I don't

think I (or the majority of parents for that matter) have settled for

less than the best for my daughters health/treatment by using the

Markell shoes, they have worked well for us.
