URGENT MESSAGE FROM DR. HARBISON

Hello Everyone - This is Salem here. I apologize for using

the listserve to post this message, but Dr. Harbison is really

struggling with trying to contact all her patients. She is down to

700 patients, but she is making a special effort to notify all her

RSS and SGA patients as first priority.

This is because even though you may know that she is moving to Mt.

Sinai, she can NOT legally take your child's medical records/data

with her to Mt. Sinai unless she has your written release to do so.

And of course, Cornell's endo department is not helping or paying

for this notification process..... So Dr. H and Yadira are having

to do this all by themselves!!! So she asked if it would be OK to

post the following request here;

FOR ANY PARENT WHO HAS HAD THEIR CHILD SEEN BY Dr. HARBISON IN NEW

YORK OR AT THE MAGIC CONVENTION IN YEARS PAST:

Please take the following steps:

1. Send Dr. Harbison an e-mail at mdharbis@... from your

current, working email address.

2. Mark the subject line as " Move Notification " .

3. In the email message, list the following information so that I

can match your child (the patient) to my list and can update their

records:

a. names of the patient, mother and father

b. date of birth

c. current address

d. current tel #'s including home, work, cell phone numbers.

e. Are you able to open an attachment and print it? It would be a

letter probably in Microsoft Word? Yes or No

Dr. Harbison will then update her records, and hit reply to your

email with the appropriate letter and release form. You will then

need to print, sign and either mail it or fax it back to her.

THIS IS TO YOUR ADVANTAGE!!!! WHY?

First, Yadira would have the correct information necessary to get

you a Mt, Sinai record number. Second, you would get your

notification before all other patients on her notification list.

Third, this would allow you to call Yadira earlier than others

before the schedule fills up. Thanks again for your help.

Thank you all for your patience, again. If you have any questions,

please forward them to ME!!!! and not to Dr. Harbison. She is, as

you can guess, rather busy....
