You can E-mail Dr.R at DrR@..., also Debbie at :DW@... and she can answer alot of your questions. Barbara is there and she is as sweet and helpful as can be and they all can help you. You can send in your packet and Fax the pap and mam also or possibly the other, thats what they told me. You can request a date or two also when you send in your pack. If I were you, I would go to a clinic if it isn't too far away and see Dr.R , this is a requirment anyhow. Then it would ease your mind and you could get a surgury date when you want, cause you will already have the clinic behind you.
