I recall people saying that it would be helpful to bring a copy of

your Patient Information Form with you to the clinic because you

will be asked all the same questions again. Which version would be

best -- the one that shows the entire narrative (I believe that's the

confirmation) or the actual copy of the Patient Information Form

(where you can only see a few lines of the narrative). Also, will I

need it again at the hospital when I am admitted?
