The topics are on page 20 of the manual (my version); they're called Required Letter Contents. There are 11 items listed -- each one is a topic or paragraph heading. Most (or all) the answers are in the manual, refer to it as you go along. Occasionally I went to Dr. R's recent paper presented at the ASBS conference because it had the most current figures or something I remembered from the website. I wrote about each subject and how I personally viewed it from my experience. My surgery was 6-2-00. Just take one section at a time and do it.
