Guest guest Posted July 21, 2000 Report Share Posted July 21, 2000 1} form filled out and e-mailed 2} pictures taken and developed for pre op photos 3} patient letter completed 4} Dr. apt monday for physical and he will write letter 5) parents wrote me a letter 6) all important medical records copied 7} have made my 10 contacts and a written summary of them Question what about the Preoperative psychiatric Evaluation is that done on the clinic visit or do I have to get one locally.... I am from out of town Have I forgotten anything...... I am at the same point only I already have the pap done. I had the Ekg done already because I didn't realize it had to be within 30 days. I have decided to wait until I have a surgery date to have the blood work done and then bring them with me to the hospital. I am just waiting on some out of town medical records to arrive. I hope to send in my packet on Monday, I hope I can get some sleep after I get my date. I am so keyed up I have a hard time falling asleep, was composing that letter in my head for weeks. You are doing the right thing by asking questions to the group instead of calling the office all the time, they are so busy and we don't want to irritate them .
