I was looking over all the things I need to have in my packet (again) and

have a question about Medical Records. I don't really have anything

significant...sore throats, cold, blah blah blah. The only things I can

think of would be my pregnancies and when I had my gallbladder out in 1986.

Does he want that record since it is far back? Thats it..other than that

haven't had any problems...any suggestions??

Holly, You must send in some medical records that shows your

medically supervised attempts at weight loss within the last 3 to 5

years.

Dr R is looking for things from the past 3 years or so. I, too, have nothing

too significant, so I asked Debbie about it. She said to only send old

records if they are pertinent to the surgery, (wt related). I sent no

records, but had my PCP mention all my past significant stuff in her letter.
