Thanks for the concern about my online form and when my insurance

letter will come. I took the advice and emailed Debbie to check

again. And yes I did make copies. I think I have 3 of the online

form without the long essays and 3 of the form confirmation with the

long essays. I also mailed my packet today!! Yeah!! What a relief.
