Oh Dinah, too bad you didn't come to clinic this week! Stormy came from

Alabama for the same reason, I bet you could have ridden with her! I would

have liked to have met you. I have BCBS of MN, and I went this week for the

same reason.. and once BCBS got the letter from DR. R saying that I had been

there, I got approved in about 4 hours! If you want, I will tell you

everything I included in the packet I had Debbie send in. Good Luck!

Dinah,

Dannielle sent me a list of what they require, but I lost it.. but I have

heard that they will ask for one thing at a time if you don't send it, so I

just sent everything I thought they might ask for in one packet. This is

what I sent: (the first 3 are things they had asked for)

Whatever Dr. Rutledge sent ( they wanted clinical notes from my visit with

him)

Psych evaluation

Nutritional counseling note from a dietician

5 year weight and diet history

A letter from my PCP stating that gastric bypass, specifically MGB, was

medically necessary and that letter stated all my comorbidities, and family

history.

Dr. R's MGB paper from the conference.

Dr. R.'s physician profile

A very long letter stating all the reasons that I need this surgery, pointing

out all the benefits of the MGB, why it is better and MUCH cheaper.

Hi Deb, Yes that would be great if you could share what Debbie sent

for you. I feel like you can never have too much information when you

are dealing with an ins company. Yes, I know I missed a good

opportunity this week for clinic with the girls from Alabama but my

little grandson was here and well I couldn't leave with him at my

house. Don't get to see him that often. He lives in Orlando. I'm

going to try and make the clinic on the July 18 if Joanne can arrange

me a flight that soon. If not then maybe I'll see Margie and her

crowd on the next Tuesday. Where is Stormy from in Alabama?

Kay,

I will be in clinic on Tues. also. What time are you scheduled? I'm not

sure where it is either. I think there is a map on the web site.

Hi anyone,

I just realized that I am scheduled for clinic Tuesday and I don't have a

clue where it is. I'll be leaving early Monday morning, can anyone tell me

Debbie,

Frona and I will be at the 1:00 clinic. I will look for the map. I was

hoping for an address also. I don't know the name of the building or

anything. I guess I might have to call Barbara Monday morning before I

leave if no one else comes up with an address. I will see you at clinic.

Hi,

Debbie (in Dr. R's office) told me that you have to go to the office first even

though the clinic isn't held there. Then, at the office you find out where to

go to get to the clinic. I asked her if I needed to be there early then, and

she said maybe I should get there around 8:30am for the 9:00am clinic.

There are directions to the office in the back of the manual.

Looking forward to meeting you all! See you Tuesday!!
