When I posted it I got several responses in my email (thanks) but have yet to see it on the board. Anyway, the docs told us that how Abby's lying during CPT (or the order of positions) doesn't matter. At our hospital they don't do trelendenburg(?) or head down on infants because of cranial pressure. However, she's now 8mos. and we wonder where the mucous is going. All the other books say exactly how to position the child to facilitate drainage. Should we do this? Also, we often use saline to loosen nasal secretions so they'll drain or can be suctioned. Can bugs grow in the over the counter stuff such as " Ocean " ? We don't want to introduce anything into her airways. Also if anyone else would like to share what they've heard or know about early diagnosed PA (as a baby) as indicating severity - also re: eradication. Abby's in my lap tearing off my right ear(laughing uncontrollably) while I peck left handed. She appears at least as healthy - if not more so than my other 2 at this age. We were lucky that it caused him no problems but many babies have reflux from being tipped on angles. I am not sure what age is recommended for beginning the different psotures to allow gravity to help the drainage. As to where the mucous is going, Abby is probably unconsciously swallowing it as it moves into her throat. Even at 16, my son rarely spits out mucous. Whatever comes up during therapy gets swallowed. It is was more copious, I am sure he would need to spit it out. If she has nasal congestion, postural drainage may also make the 'upside down' positions (as my son calls them) less comfortable as it may cause increased pressure. The saline is an excellent idea, but yes the over-the-counter types can grow pseudomonas and other bacteria once opened. They are even more susceptible to growing bacteria if you release the squeeze on the bottle before removing it from the nose. Perhaps we are overly cautious, but the only saline we use is the sterile, individual, 5 ml.vials of 0.9 per cent Sodium Chloride Inhalation Solution, designed for respiratory therapy (says that right on the box). I have to get the pharmacy to order it in and it comes in a box of 100. You wouldn't be able to mist it into her nose like a squeeze bottle but it could be nebulized through a pediatric mask or used as drops right from the plastic vial. I left the nasal aspirator sitting for 2 days before I cleaned it and I was rinsing it out and TONS of mold (black, icky, slimy) stuff came out of the tip. Very nasty. My son hasn't cultured anything--it is just the natural bacteria that was growing. So be very cautious about cleaning anything that goes in the nasal passage. We ended up quiting as it was too hard on her due to reflux. Mostly she swallowed what ever mucus came up. We also use a saline spray and I just regularly buy a new bottle, especially during and after sickness. We bought a heavy duty one called an ulcer syringe that is actually used in surgeries. We microwave it to steralize it.
