Here's what I went through:

1 - I was tested at a naturopath to determine the order in which they

had to come out.. (The leaching of metals is aggravated by the electric

charges generated by the arrangement you have in your mouth. It " s been a

while, read the book rather than rely on me though :)

2 - Brought those test results to the dentist who decided on a different

order (Did all upper-left first for example, instead of a cross-mix)

3 - I still survived pretty well even if we didn't rigidly follow the

order :)

4 - During replacement or extraction, the process was to first swallow a

few charcoal tablets

5 - Placement of a dental dam to prevent any swallowing of the extracted

materials

6 - Oxygen mask for the whole duration, for themselves too

7 - Thorough vacuuming of the whole process in a special system

8 - Vitamin C tablets were given to me for later at home
