I found on Amazon, but also available at http://www.orthognathic.com/ Inital thought is $64 is a bit steep for a 120 page paperback. And VERY goos to give anyone looking after you. Because whilst you can get all the information here, most often you might have communication issues after surgery. I would buy that.
