There's some good lists of questions on the info website I set up at http://www.geocities.com/orthognathicinfo/ Check under SURGERY FAQs, and RECOVERY FAQs. I always made sure I had at least 2 questions ready for my ortho each visit. My surgeon got a lot more at my pre-op visit (I think there were 22 questions), and at each subsequent visit (11 at the first post-op, which my husband had to ask as I didn't want to do much talking). The number diminished at each subsequent visit until I ran out. If it's a real burning issue or something that you think's gone wrong and aren't sure, by all means, telephone them (especially post-op), but otherwise, I'd keep the questions to your visits. With the ortho, make sure you ask them BEFORE he starts working in your mouth - it's hard to ask while they're working, and my ortho usually left the room before I got a chance to ask questions. It was under files and dammits surgery questions I think. It had lots of detailed questions. My os was so impresses by all the questions I asked he asked if he could make a copy. It help to reassure me before surgery the asked everything there was to ask. So when the day of surgery came I was well informed and relaxed. Everything truned out how I expected and I also knew what to expect from this site.
