It's a metal device that is attached with those metal bands on your molars. I had it too. I got the expander in, then had the SARPE where they made some small cuts in my gums in order to be able to activate the expander. After that, we had to turn the key for one more week (starting about one wk after SARPE surgery). You will get a pretty big gap between your 2 front teeth. After about after a month, your orthontist will start putting the teeth back in their place. You'll be surprised how quickly the gap will close. The expander's a bit of a hassle for the first weeks, but you do get used to it. I had it in for 3 months. You should ask your orthodontist the specifics, are you getting the expander in so that they will do the surgery during your lower and genio? That's what they did with me (everything together: SARPE, lower advancement and genio). It's also possible to have the expansion first, and then after you've been fully expanded, do the lower and genio in a seperate surgery. Any other questions, let me know. There has been no mention of a second surgery though so I guess that means the surgeon will do that at the same time as the rest (I might as well ask for my appendix out at the same time the rate I'm going - upper,lower, genio, wisdom teeth and now SARPE too!). Do you think my orthodontist will fit the actual expander when I go to see her in a fortnight? My surgery is not until 1 February but I know I'm going back because of this palate expansion business. Does it hurt when you have the expander fitted and when you turn the key?? And can you feel the expander with your tongue? There's going to be no room left in my mouth! It was on the roof of my mouth and I had to turn it about a quarter turn each day with a key. But it didn't work. But I didn't have any cuts in my mouth either. Hmmmmm....I told the OS about it and he said that those don't work for people my age (43) Hmmmm....but then again, like I said, I didn't have cuts in my mouth. I guess each case is different, huh? Also, fyi if you're reconsidering a rhino, I'd not think about a nose job during this surgery, since your nose will get a bit wider because of the expansion of the pallate. So you'd really have to see first if you like the end result after all the turning of the key, if you would decide on a nose job! It doesn't hurt to have the expander fitted. You can definately feel it with your tongue, it's probably going to hurt your tongue for a while. Ask your ortho for that numbing gel, that helped me, and wax also. It's going to feel like there's no room left in the beginning, and you're going to hear it in your speech, but you will get used to it. By the way, if you're doing the SARPE together with the rest, I don't think you'll also have a separate upper (where they cut it horizontally). If so, you'd probably get a 3 piece leforte instead of a SARPE. With the leforte, they can cut your upper jaw in 3 seperate pieces, which would mean you wouldn't need an expansion with the SARPE, because they can reposition the 3 pieces back wider (I'm not sure about this though! so please ask your doc!). Please check with your doc what your procedure will be for your upper jaw to be absolutely sure and check why he wants to do things a certain way, so you'll know what's the reason.
