Guest guest Posted November 11, 2004 Report Share Posted November 11, 2004 Guess what I'm getting for Christmas this year..... a chin.... a real chin, made out of bone and all the way forward where it's supposed to be. I'm having a genioplasty procedure that my doctor calls a " sliding drawer " , also sometimes called a " box " genioplasty that will allow for the 10-12mm that I need to have a normal profile. This will represent my second genioplasty as I had a standard sliding genioplasty along with lower jaw advancement ten years ago. That surgery gave me a total advancement of about 10mm, but that really only halfway corrected the profile problem. I had an implant put in this fall but it failed to heal properly so was removed already. I've consulted several surgeons on doing a secondary genioplasty and although they all agree that it can be done, this option looks the best for really delivering the look I want and need. Although I'm an engineer by profession, I am also getting into modelling, so having a good profile is literally money in the bank in that business. My surgeon has a very impressive portfolio of jaw and chin advancement cases that are some of the best transformations I have ever seen, so I'm certainly hoping that his artistic and surgical capabilities will benefit me as well. I have sleep apnea and I was recommended to get a lower jaw advancment and genioplasty to address this issue. I have since discovered less invasive procedures that I am considering. The problem is that these less invasive procedures do not improve my overbite/underdeveloped chin. So, in short, I will likely resolve my sleep apnea with these less invasive procedures but not improve my overbite and recessed chin. In your opinion: do you think the lower jaw advancment OR the genioplasty had the greatest affect on the improvement of your lower profile? I could get the less invasive procedures for the sleep apnea and get the genioplasty alone to address the recessed chin. I am just concerned that the genioplasty (12 mm) without the lower jaw advancment would produce BAD results? Please also provide the name of the surgeons you are using for this " sliding drawer " procedure. I would appreciate the feedback. My doctor said I need about 5 mm forward, and 3 mm down (I have three dimensional deficiency, short vertically and horizontally). But one thing he told me is that with greater advancments there is two issues. First the numbness might be more pronounced since the nerve could be stretched a lot. The other is the " step-type " irregularity that arises when they gut a piece of the bone and move forward (like an open drawer). I personally dont want to have this step type thing so I insisted to keep the osteotomy less than 5 mm. So keep consideration of the risks as well before extensive movements.
