As someone who has made the monumental, life-changing decision to undergo a

Weight Loss Surgery (WLS) procedure, we greatly value your opinion on matters

concerning the pre and post-operative patient. As you are aware, there are not a

lot of information resources for the pre and post operative WLS patient.

Our goal with this survey is to get a unique understanding of the informational

and resource needs of the people that count. you. We would appreciate it if you
