I'm thinking my ortho has an installation fee of $800 or so, just for putting the appliances in place. I would (if I may say so) think twice and ask a lot of questions about figuring out what you want about those spaces, rather than asking what the docs recommend. You may be exactly on your target, but they have studied this stuff and usually have reasons for what they suggest. I suspect that they need to get the teeth in the best possible state for surgical alignment before you have surgery, but that's totally a guess from a laywoman. I have seen such amazing surgical stuff on the teevee that I would never say that any doc could not do anything, but again, you need to be asking the experts, the trained people, these questions.
