I find it difficult to believe that there would be no difference in outcome between one or the other – either your upper jaw is deficient or your lower is excessive, or both. I would go back and grill your surgeon and ortho and if you haven't, get a second opinion from another surgeon (I'd do this regardless). I had a combination of both done, upper slightly forward, lower slightly back. My surgeon felt that doing lower only would give me a weak chin and leave a lot of loose skin to deal with later. I am very happy with the results and would not do it differently if given the chance again. I wouldn't begin to presume to make a suggestion. I think (probably because I had such an excellent outcome with lower advancement) that the lower alone is probably an easier surgery. But it can carry a risk of numbness. Don't they have a recommendation, or some benefits and drawbacks to offer you? I swear, I'd tell the surgeon you're confused, and holler for another consult with him, preferably with the ortho involved as well, either in person or in a phone consult.
