I noted with interest the statement under " Treatment of Ocular cea " that " low-dose treatment with oral isotretinoin (10 mg, three times weekly for two to threemonths) has also been successful in recalcitrant ocular cases, " a clear reference to Kligman's suggested regimen. I discussed this regimen with my current dermatologist just two days ago. Surprisingly, she was willing to prescribe 20 mg daily; but she refused the notion that 10 mg every other day could help my ocular rosacea. Can you--or anyone else on the board--suggest why she believes 20 mg daily would be better than the suggested regimen, especially when its intended only to treat my ocular symptoms? I filled the prescription, but I have delayed taking any of the capsules because of the statement in Nase's book that dosages of accutane other than those which fall into the very-low-dose can actually aggravate rosacea. The pharmacist who filled my prescription stated that 40 mg of accutane is considered a high dose; 20 mg a moderate dose; and 10 mg a low dose. The last thing I want is to aggravate my rosacea. I guess I'm asking what I've asked before: Does anyone know of a dermatologist in the United States who believes in Kligman's suggested regimen? I'd certainly like to find that person, if he or she exists. My guess, based on statements made by my dermatologist, is that Kligman himself is still alive but my internet searches have turned up relatively close to empty. I was told about a Bernard Goffe in Washington state, but my current understanding is that he also believes in the 20 mg dosage over the 10 mg dosage for ocular rosacea. I've been taking 2,000 mg in divided doses daily for nearly two years and have seen ocular rosacea symptoms diminish significantly. I eat one or two crackers before taking the tetracycline on an otherwise empty stomach, and seldom have an upset stomach. Once in awhile, I'll eat some yogurt just to make sure I reintroduce some good bacteria in my gut. However, I dislike most kinds of yogurt and think this issue is overstated, anyway, so I'm not too religious about yogurt. As for safety, I've been taking tetracycline in varying dosages for most of my adult life, the result of late-onset acne. It's one of the most benign medications I've ever been prescribed. The worst experience I ever had was when I forgot to apply sunblock and suffered a nasty sunburn. That only happened once. (Reduce chances of it since less inflammation ? ) by the way,i never had any problems with flaky skin till after i went on minocycline for 10 days in june.
