My husband and I have been trying to research our options about treatments and physicians in the Dallas area. We live in McKinney. We would like to try the Ponseti method first, and would like to begin the casting process as soon after birth as possible. From what we've been told, the Ponseti method is not used at ish Rite. Do you know if this is true or not? I have also been concerned about using ish Rite due to the fact that the application process seems to take a little while. I'm not sure if they would accept an application for our daughter until she is born, and as I mentioned earlier, we do not want to delay her treatment. How quickly after birth were you able to get treatment for your child? It is my understanding that treatments are provided free of charge to the patients of ish Rite because the facility is fully funded through donations and fund raising. Have you found this fact to be true? The financial benefit seems great, but we don't want to compromise the type of care (treatment method)for financial reasons. He did use the Ponseti Method on my son. He is not on Dr. Ponseti's list of qualified docs, but he did a wonderful job on my son's feet. He is now 2 1/2 years old and very active. If I didn't tell you, you would not know he had clubfeet. If you go to the hospital's web site you will be able to find his name. Good Luck on everything, I don't have any answers to your other questions, just remember this is your child and be presistent with doctors and their staff. Because let me tell you if they were in the same situation they would want the best care possible and that is all you want I am sure. Just as a side my son didn't get to see Dr. Podezswa until he was 3 months old. We saw another doc first who did not correct his feet and was talking surgery. I hope someone can give you personal experience and info on docs in Texas and/or ish Rite. I recall that, at least as of several years ago, ish Rite was a center which was using the French Method of clubfoot treatment. An article on their results with that method was recently pubished in the journal of Pediatric Orthopedics, showing that a fairly large percentage ended up needing surgery.. here's the abstract, taken from Pub Med: J Pediatr Orthop. 2005 Jan-Feb;25(1):98-102. Unfortunately, even though the Ponseti method has become generally acknowldged as the way to go, there are quite a few regions of the country in which it's frustratingly hard to find doctors who use it, or use it well.. even highly populated ones with major medical centers. Many of us have ended up having to travel for treatment as a result. I want you to know that, even though that sounds daunting, it IS possible to travel to get treatment, IF you are unable to find it nearby and the Ponseti method is what you want. I'm sure that's hard to feature at this point .. but there have been many parents who have gone to Iowa (even from overseas) with a newborn baby, even with older siblings .. stayed for a couple of weeks at the Mc House adjacent to the hospital.. or flown in for appointments over the course of a few weeks.. it's not as impossible as it sounds.. esp. when you consider the alternative.. going through months of treatment which might not work. (there's a mom here who drove all the way from Texas to Iowa for her son's treatment..) I recall that there might be a doctor named Gottfried in Texas who uses the method, but I can't say for sure.. You could call or write Dr. Ponseti's office to ask for a recommendation for you. The contact info is at the VH site. I hope you are able to figure out a good situation nearby! I too live in Texas and although i personally had to take my daughter to see Dr.Ponseti, there is a girl in this group () who told me of Dr.Kahn and Dr.Shapiro but they are in Austin. Both are Ponseti trained but have not yet been added to the list. I don't know how far that is for you but just thought I would let you know. I believe treatment begins soon after birth. My daughter was born in the evening and by the next morning was casted. Their results, as reported at a 2003 professional meeting, gave about a 60% chance of avoiding the major release surgery regardless of the method used. However, most other centers report a 90+% avoidance rate with the Ponseti method (the 60% rate for physiotherapy is similar to reports out of other centers). I know of at least one parent here whose child was treated using the Ponseti method at TSR. With terrific results. Hopefully she will see this and chime in. I don't know anything about how long it takes to get an application approved and all of that, but I imagine they have a streamlined process for clubfoot patients. Hope this helps a little. Guest guest Posted June 23, 2005 Report Share Posted June 23, 2005 Hi Amye, I live in Mansfield and my son Jake is being treated at ish Rite by Dr. Rathjen. Jake was born 8/1/04 with bilateral CF and we got his 1st casts at 1 week and 2 days 8/10/04. I had done research online (although I didn't find this group until a few months ago) and already knew that I wanted to use the Ponseti method. When we went that 1st day, they gave us the choice of the Ponseti method or the French method. The Ponseti method involves the casting and the French method is very labor intensive with physical therapy but no casts. Being a single working Mom, I couldn't go over there every day which is required for the French method. From what I could tell they followed Ponseti's method very well up until the DBB (the braces and shoes). They had us wear it full time for 3 months then reduced his hours to nights/naptime only 12-14 hrs/day. This is where our problem started. When his hours were reduced, he began screaming at night when I would put them on. After 2 weeks of no sleep and the orthopedic dept at ish Rite couldn't find anything wrong with the shoes, I got online and found this group and told them the problem. I was informed that his hours were reduced too much. I went back to full time then gradually reduced his hours down to 18 hrs a day and Jake has been fine ever since. This is where I found out that they were using a modified Ponseti method. Ponseti protocol says 3 months full time in DBB, then 18 hrs a day until they start walking, then 12-14 hrs a day. Evidently they think 18 hrs/day slows development - but Jake crawled and pulled up at the same age as my first non-cf child. He crawls and pulls the same whether he has the brace on or not - he's now nearly 11 mos. I think his feet look great although just for peace of mind, I want to take pics and send them to Dr. Ponseti. From what I've read on this board, if his feet weren't corrected, they wouldn't be staying in the shoes and I don't have that problem. So in summary, I think they did a good job - won't know for sure until Jake starts walking (Dr. Rathjen said there's still a possibility of surgery if he doesn't walk right - but I'll take Jake to Iowa before I'll let anyone do surgery on him). The only problem I had so far was the difference in hours for the shoes and brace. Regarding, the application process, I had no problems at all. They sent me a package - I can't remember if there was paperwork to send them beforehand or if we took it with us at our 1st appt. Guest guest Posted June 23, 2005 Report Share Posted June 23, 2005 Hi Amye, I live in Mansfield and my son Jake is being treated at ish Rite by Dr. Rathjen. Jake was born 8/1/04 with bilateral CF and we got his 1st casts at 1 week and 2 days 8/10/04. I had done research online (although I didn't find this group until a few months ago) and already knew that I wanted to use the Ponseti method. When we went that 1st day, they gave us the choice of the Ponseti method or the French method. The Ponseti method involves the casting and the French method is very labor intensive with physical therapy but no casts. Being a single working Mom, I couldn't go over there every day which is required for the French method. From what I could tell they followed Ponseti's method very well up until the DBB (the braces and shoes). They had us wear it full time for 3 months then reduced his hours to nights/naptime only 12-14 hrs/day. This is where our problem started. When his hours were reduced, he began screaming at night when I would put them on. After 2 weeks of no sleep and the orthopedic dept at ish Rite couldn't find anything wrong with the shoes, I got online and found this group and told them the problem. I was informed that his hours were reduced too much. I went back to full time then gradually reduced his hours down to 18 hrs a day and Jake has been fine ever since. This is where I found out that they were using a modified Ponseti method. Ponseti protocol says 3 months full time in DBB, then 18 hrs a day until they start walking, then 12-14 hrs a day. Evidently they think 18 hrs/day slows development - but Jake crawled and pulled up at the same age as my first non-cf child. He crawls and pulls the same whether he has the brace on or not - he's now nearly 11 mos. I think his feet look great although just for peace of mind, I want to take pics and send them to Dr. Ponseti. From what I've read on this board, if his feet weren't corrected, they wouldn't be staying in the shoes and I don't have that problem. So in summary, I think they did a good job - won't know for sure until Jake starts walking (Dr. Rathjen said there's still a possibility of surgery if he doesn't walk right - but I'll take Jake to Iowa before I'll let anyone do surgery on him). The only problem I had so far was the difference in hours for the shoes and brace. Regarding, the application process, I had no problems at all. They sent me a package - I can't remember if there was paperwork to send them beforehand or if we took it with us at our 1st appt. You could always email Dr. Ponseti directly and see if he can tell you anything about Dr. Lund. Also, I wanted to tell you that while it is important to begin the process shortly after birth it is not so urgent that you should miss out on those first few days cast free snuggle and bonding time. Many moms have said that if they had it to do again they would wait at least a few days or even a week or two to start the casting just to give them the time to enjoy their newborn crooked foot and all. It gave us a couple of weeks to adjust to a new baby, get nursing off to a good start, rest from the delivery and just enjoy our new bundle just the way God gave her to us. I know it's weird, but I kind of miss her crooked foot since that's who she was when she was born. Kind of like the beginning of them outgrowing the newborn period. We took lots of pictures too!
