Reducing the hours in the DBB's

Hi Everyone,

I havent posted in aaagggges as keeps me on my toes these

days, he's 9 months old and is crawling all over the house with his

DBB on and twice he has pulled himself up to stand with the DBB's!!

He has been wearing the DBB's since he was 4 onths old (following a

heel release surgery- he's got bilateral talipes)and did 3 months of

24/7 wear. We had a check up in March and his hours were reduced to

16-18 hrs of DBB wear per day, which we are still following 3 months

later.

We have an appointment on the 27th to see our specialist here in

Sydney and im hoping all is good. At his last check up in March he

found that the flexibility in the left foot was not as good as the

right foot and if it wasnt improving they may have to repeat the

heel release on the left foot. Fingers crossed that all is well, but

i have a feeling he may need the heel release on the left foot

again.

We had some very restless nights 2 months ago when he would wake up

about 5 times a night by pulling his feet out of his shoes (hadn't

done this since the first week or two we got the DBB's!) so we took

him to his physio to see if the shoes were getting too small but

they were fine. She suggested putting them on without any socks and

we tried it that day and it was like MAGIC!!!! No more screaming and

they stayed put despite his trying to get them off!!

My question is, whats the next stage of hourly reductions after 3

months of 16hr wear? 14 hrs? 12hrs?

Hi,

I noticed no responses to this, I hope I am not repeating something here.

The way I figured it takes into account that the child would be at or

around 14-16hr/d prior to walking. So, if you take the average age of

first steps at or around a year and divide that up amongst the ranges you

decrease to over that time, it's about every 3 months. Which for us

correlated to our visits to the Dr. for checkups. So I would think that if

everything is perfect with the foot that you'd do the reductions at or

around each 3mo appointment which would leave your last reduction to

14-16hr/d at or around 1yr. Then you stay at 14-16 till they're

walking. Walking usually means you can go to the 12-14hr/d till they're

released from the brace at 3-4yo.

That's just my perception based on how it worked for us time wise with a

newborn. Older babes may be walking much sooner than the 3mo schedule

would allow. For it to work out about perfect with 12-14mo as described

above only applies if babe is cast and released to the FAB within the first

2-3mos. I would still default to *around* every 3mo to get an idea of how

it'll go if that's the schedule the doc sees babe at.

Nisha, I somehow missed this post over the last week. From an email

from Dr. Ponseti that Stella received (January 2005), here is what

Dr. Ponseti said with regard to the FAB schedule:

" After 3 months of full time brace wear, we usually gradually wean

the babies out of the brace, starting with 4 hours a day out for one

month, then 8 hours a day out. We try to have the babies wear the

brace at night and naptime for a total of 16-18 hours and once they

are walking wear it at least 14 hours. We used to recommend bracing

continue to age 3, but since we have seen some relapses, we now

recommend 3 1/2 to 4 years of age.

Thanks for that Daiga!

We had our specialists visit last Tuesday and he was happy with the

flexibility in both feet. He said that we should monitor that left

foot, but it was good and he was able to get it into the neutral

position. The Dr. advised us that when he starts walking or bearing

more weight on his feet, this will naturaly help the muscles strech.

So unless we get really slack with the DBB's in the next 3 months

(not likely!!) he wont need repeat surgery on that left foot.

Yayyyyy!!

He also told us to keep up the 16hr wear and we are due for another

check-up in September.

So good news so far- no need for another heel release. Must be doing

something right!! Phew!
