Between my hearing loss & problems with memory/concentration, I'd think "maybe she did tell me." But to eliminate confusion & manipulation, I started a communication notebook. When she told me something important or asked to do something, etc., it was her responsibility to write it down in the book & have me sign it. If I signed it without reading it, or read it & forgot it, that was my problem. I also have a signature that is hard to copy, so that never was an issue.
