Letters TO Primary Care Physician

Hi All...

I'm kinda new...kinda lurking. One of these days I'll be able to

keep people's names/responses straight! I'm 31 and live in

San Francisco. I'm preparing for the possibly torturous journey

towards getting approval for a DS.

My beloved doctor of eight years moved back east last month. I

am sooo disappointed since I had a wonderful relationship with

her. I was able to have some brief discussions about WLS with

her prior to her leaving, so at least it is already in my chart. She

said that when I needed the referral to just call and get one...just

like that. Unfortunately, it was a week before she left and I hadn't

contacted the surgeon's office yet.

I have an appointment with my new doctor on Wednesday to ask

her for a referral. I don't know what she'll say, so I'm trying to

prepare a packet for her of articles and other information. My

question is: does anyone have or know where I can find sample

letters to doctors explaining the need for my surgery?? I know

I've seen a few before, but I guess I forgot to bookmark it. I

figure I'll write her a letter with everything in it so she can use

it, if necessary, in conjunction with my records in case a) she

needs a little convincing or we need it for insurance. I just

don't know where to start. My drafts so far are starting to look like

frustrated journals.
