For some reason alot of the approved docs have gone to telling the parents

this. Dont know if it is for compliance or what, but they are doing a

disservice to the babies.

Dr Ponseti goes something like 23, 20, 18, 16 ... at that point you are

probably at all sleep hours. I cant remember exactly how the times drop,

but someone will. Eventually you make your way to 14 hrs and stick with

that for some time.

correct? I always heard that they have to wear it at naptime too?

Wow- Our Dr. (Ponseti approved) had our son wear his shoes 23/hrs until he could

pull himself up to a stand w/ the shoes on... that was at about 9 months old. I

would check on that w/ Ponseti.

Wow- Our Dr. (Ponseti approved) had our son wear his shoes 23/hrs until he could

pull himself up to a stand w/ the shoes on... that was at about 9 months old. I

would check on that w/ Ponseti.

Here's a copy of a note from Dr Ponseti regarding the bracing

protocol. I don't think very many doctors follow it closely. From

what I've seen here, most doctors (even ones who are on The List)

tell parents it is fine to go straight to " night time only " after the

3 months at 23/7. A few doctors (Dr Noonan in Madison, WI for

example) have babies stay at 23/7 until they can pull to stand.

Parents, of course, can choose to do what they want. I always figure

it's so much easier to put in the longer hours when they are

smaller. You could take this to your doctor and ask him why he is

not following this more gradual schedule.

The note from Dr Ponseti, which follows, is also at message #36853.

The length of time the child wears the brace depends on the age of

the child and the stiffness of the foot. There is variability in

the time in the brace. In an infant, usually the brace is worn

full time for three months, then 20 hours for a month, this 16-18

hours for a month and then at night and naptime. Once the child is

walking it can be worn 12-14 hours a day. If the foot is very

stiff, the brace may need to be worn more hours and if the foot is

very loose, fewer hours. However, since we have seen late

relapses, we now encourage parents to have their children wear the

brace to age 4 years.

I agree it is a dis-service. Dr. Ponset prescribed we wear the shoes 23/7 for 3

months, then down to 18 until he was walking? Then to 16, gradually down to 14.

Myself, I would go this route, not immediately to bed time wear only - for two

reasons: First, the Master prescribes it this way. That is reason enough. But

secondly, a few babies on this loop have gone the full-time to bed-time in one

jump and suffered some bad transition periods where the parents had to go back

to the 23 hours and gradually " wean " them off the shoe. In any event, saying

" bed time " is too vague. Aim for a minimum of 14 hours of wear a day for a

loooong time to come.

I am sure you will get a few replies to your question as this is a

hot topic. From what I can remember off the top of my head (from

reading the global health book and from this site), Dr. Ponseti

usually has the child wear the brace 23/7 for roughly 3 mos then

reduces hours to 20 then to 18 then to 16 and then to 14. 14 hr wear

is for a child that is walking. I may not be 100% accurate here but

close.

We were directed by our doctor - Dr. Pirani - who comes very very

highly recommended by Dr. Ponseti (he has said that he recommends him

1000%) to have our daughter wear the brace 23/7 for 3 months (after

her 4 casts). He then examined her foot and reduced our wear time to

14-16 hrs. He said that once she was cruising that we could reduce to

12-14. I do not want to say that Grace has a 'mild' case but her foot

did have a great deal of flexibility and good dorsiflection, so much

so that she did not need the tenotomy. He said that he prescribes

hours based on the individual foot and that we should always try to

do the maximum hours, not the mimimum.

Personally, what your doctor has said would not send up any major red

flags up for me. If you have been on this site for a while, I am sure

you have learned tons of information. If everything seems right with

your son's treatment, I personally wouldn't worry. Do you mind my

asking who your dr is? And has your son tolerated the 23/7 well (if

so one would assume that he has received good correction).

I myself would just keep the brace on a few hours longer. It is also

important for your son to wear the brace while he is awake as well so

that he can learn to pull up and sit down in it, he will need to

learn this skill so that he dosen't hurt himself in his crib. You

will get a real kick out of some of the things you will see him do in

that thing! 5

We used to put the brace on a couple of hours before Grace went to

bed and then kept it on for a couple of hours after she got up in the

morning. I never bothered to put it on her for naps and just got the

whole shebang done at night, it's hard enough to wrestle that thing

on a busy baby once a day lol.

My doctor told me this as well, and she is Ponseti approved. We started out at

18 hours and then slowly reduced it for the last 5 months. He now wears them

from 14-16 hours a day. He is 10 months old.
