Guest guest
Hello, I just spoke over the phone with Dr. Pirani who came highly recommended by Dr. Ponseti and by lots of you here. He is very nice and said that I could definitely come to Vancouver to have my baby treated by him. He did say that there were two doctors in Calgary at the Children's Hospital that I should consult with before I decide to travel all the way to Vancouver to see him. Has anyone heard anything about Dr. Kiefer and Dr. Joughin? He says that he knows them and knows they use the method but doesn't know how many feet they've treated or their success rates (he said that if they have done at least 20 feet using the method with good success rates, that I should consider them). Apparently Dr. Kiefer was trained by Dr. Pirani. I will definitely go and see both of them. I actually had a consult booked with a Dr. Goldstein and he recommended talking to these other Dr. instead. I definitely will, armed with my list of questions! He said that if I come out here he casts every Tuesdays so every 7 days. Dr. Ponseti casts every 5 days and that is supposed to be ideal but sometimes it can fall on weekends etc so in Vancouver he does every 7 days. What did all of your Dr.s do? Also, since my baby is due Dec 18, he said I could delay the first casting for a few weeks and commence in the New Year. I think I'd rather start right away or at least get the doctor here to do the initial casting until I can get to Vancouver. Isn't it better to start right away? I am still considering going to Iowa or Vancouver but it sure would be easier to stay in town since we've got a 13 month old and a 2.5 yr old to content with. I just feel anxious knowing how important it is to start out with the right doctor and don't want to mess it up! I could just fly out to Vancouver every week but that is lots of $$ and I do have an aunt there and can probably stay with her. I wonder if an airline would give a deal on tickets if you explained why you had to fly so much? Probably not, but you never know. Sorry to ramble I just basically wanted to know what you all thought about 7 vs 5 day castings and if anyone has heard of either Dr. Kiefer or Dr. Joughin? Thanks! Guest guest
Hi Halley, First off, the casting every 7 days is actually the norm. And I think even Dr. Ponseti does 7 days in Iowa for in town patients. Those from out of town get an accelerated casting schedule of every 5 days so they can get home faster. It cuts off a couple of weeks from the normal casting period when done every 5 days. But most docs do cast weekly so that's not something you need to worry about. As far as when to start casting, it depends mostly on your birth history I guess. If you expect to go 2w early, then waiting would mean 4w. If you are always late, or even on time you probably won't need to worry about it. Casting early is beneficial, but not so important that it can't wait a week or two. Also, that is a wonderful time to be with your infant with his/her feet exactly the way they are, all twisted and sweet... you won't see those feet ever again and so many of us who jumped right into casts after birth feel that we'd like to see those feet again and love them a little longer than we got to. It's perfectly ok to wait a week or two to start casting, enjoy that time and take lots of pictures. You will value them later. It is actually recommended that you wait a week to be able to have that time with your baby. One more isn't going to make the least bit of difference assuming the hands that are treating those feet are doing it properly. You may not have seen this, but recently Dr. Ponseti has been able to obtain an impressive amount of correction on a 3-1/2yo's previously untreated feet. You can see their story if you read back just about 4 days, they are still in Iowa right now. Simply amazing! The standard has gone from needing to get treatment preferably before 1y of age to before 2y and now we're seeing much older feet respond in ways they hadn't previously thought possible. Take your two weeks... take 3... make sure the doc is the *right one* for *your* child, keep your eyes and ears open and don't doubt your research. Be strong in how you want your child treated and post any time you've got questions. Smooth sailing from her on out... well, gotta get the baby our first lol! Lastly, I absolutely would consult with the docs in your area. Especially the one Dr. Pirani trained. If you go in with your guns loaded and make sure he knows you " talked " to Dr. Pirani... I'm sure that'll make an impression of sorts hehe... The thing is, you know what to look out for, you know what you want, and you have the ammunition to make an impression before you even walk in the door. If you find it doesn't work out, you can always find another doc. You may want to make appointments with both of them, say a few weeks apart so that if you do need to change you don't need to wait. I know I could do this in the US, but not sure in Canada. If you could though, wouldn't be a bad idea for just in case. Thanks for posting, it would be great if you did find good docs in your area. We're always looking to hear about great docs we didn't know about. Then again... it's hard to want to be the guinea pigs when it's your child isn't it? tough... Enjoy the rest of your pregnancy (as much as you can with those other two running around - boy do I know that!) and do let us know about those docs. Hopefully someone here will be able to tell you something more. I do remember hearing the name Keifer recently though... hummmm... try searching the group archives for those names and see what you come up with maybe? Guest guest
I second all this! Do take a little time to let your baby adjust to " being born " and you two bond and you enjoy those little feet and don't fret about any of it! I realize religious posts here are sort of prohibited but I just want to say I was reading some scripture quotes last night about pregnancy... how God knows our children in the womb as he creates them in the image they need to serve the purpose He already has planned out for them in life. Take comfort in your crooked baby that God doesn't make junk. s. Re: chatted with Dr. Pirani, can I ask all of you a couple of things? Guest guest
Hi - I just had my baby on August 4 and remember wondering how soon we should get her treated too. In the end, we flew out to Iowa City and got her treated at 3.5 weeks. I called Joyce (Dr. Ponseti's secretary) to ask her if we should fly out right away, and she told me that we should enjoy her and it really wouldn't make a difference to come at 2.5 weeks or 3.5 weeks. So, we went at 3.5 weeks. Good luck and enjoy your pregnancy! -Yuuko and > >Hello, > > > >I just spoke over the phone with Dr. Pirani who came highly recommended by > >Dr. Ponseti and by lots of you here.
Guest guest
Hi Halley, First of all, congratulations on your pregnancy!! And welcome to the group. We see Dr. Pirani in New Westminister (Vancouver) and he is just excellent. We have also seen him train quite a few doctors in our short history with him. He has also been intregal to bringing the Ponseti Method to Africa by being a key member in the Ugandan Project, he is just an amazing passionate doctor!! If Dr. Pirani trained Dr. Keifer, I would give him a shot first, before you travel all the way out here. You know the check list and what questions to ask - you are well informed. If after speaking with him and asking him some of the questions from the checklist (files section) and you feel comfortable, I would see him. If you are not comfortable with his answers, results etc, you can always travel out here to see Dr. Pirani, his casting clinics are every Tues and they will always get you in right away. If I had it to do over again, we would have waited longer to get Grace into her cast (a couple of weeks). Just to snuggle her and her crookedy little foot. We would have also taken many more pics!! As it was, she was undiagnosed (had a kidney issue, so her rcf was missed) so it was such a shock and without knowing it was OK to wait a couple of weeks to get her into a cast, which it is, it is totally fine, we had her casted at 4 days old. Anyway, good luck with your appts. and keep us informed! & Grace ) 14mos right unilateral FAB 12-14hrs > Hello, > > I just spoke over the phone with Dr. Pirani who came highly recommended by > Dr. Ponseti and by lots of you here. Guest guest
Hi Halley, I can't help with the specific Drs, but I can tell you my experience on the other questions. Raphael (3 weeks old today, and in his third cast) is being treated by Dr. Mosca (in Seattle), certified by Ponseti and highly recommended by folks here. Our first cast was at 9 days old (9/21, Raphael was born 9/12). I had initially hoped to start sooner --- my EDD was 9/5, and we actually had an appointment scheduled for 9/9, which we didn't make :-P. In the end, I'm really glad we had the time to settle in first rather than contending with the cast (and slighly more complicated diaper changes, plus a fussy baby on cast day) right away. Most of our cast appointments are 7 days apart, just this one was after 5 days. Dr. Mosca actually said something today about possibly slowing down (doing less correction with this cast than could be possible) to give Raphael's feet time to get big enough for the shoes before he's done with the casts! So, I guess that mean that neither the 9 day delay nor the weekly casting schedule is slowing things down, at least. -- Toby 5/02 Raphael 9/05 lcf, 3rd cast
