Guest guest Posted October 20, 2005 Report Share Posted October 20, 2005 Basically you need a letter of medical necessity written by the prescribing doctor, a written prescription for a life time wear of the Ponseti abduction brace and custom AFO(shoes) to maintain correction, and then an invoice from and a claim form you can print off the BCBS website or call and have them mail you one. Fill the claim form out(make a copy for you), send copies of the letter and prescription, not originals, of all the above and wait for them to process the claim. It took about 20 days for them to process and send us a check. However, this could apply to deductibles if your child has not met the yearly deductible. You would have to check your plan's benefits. That is it in a nutshell. It is important and crucial to follow the steps or it will be denied.
