Just curious, who is the doctor treating your son? Is he/she following the Ponseti method? > > Hi, I'm new to this group. Guest guest Posted October 26, 2005 Report Share Posted October 26, 2005 Amy, Often there are problems with the shoes staying on right if the foot is not fully corrected. It is important that the foot be able to dorsiflex a good 10-15 degrees (almost as much if not the same as his other foot); if the tenotomy (the tendon lengthening) did not achieve this, then his foot is not ready for shoes and will not stay in them. Also, in a fully corrected clubfoot the final cast (the one that is put on when they do the tenotomy) the foot should be turned out to a 70 degree angle. This is essential for a few reasons. 1) The left shoe (attached to the bar) should also be set at 70 degrees and if angle of the foot does not match this will cause discomfort and allow the baby to pull his foot out. (The non-clubfoot is usually set anywhere between 25-45 degrees.) 2)The foot should be turned out to 70 degrees in order to achieve full range of motion. Much like his right foot, the left should be able to turn outward even beyond 70 degrees; if it is not able to do this, the correction is not complete as he needs to have that normal range of motion. 3)Also, related to the full range of motion, is that the clubfoot has a tendency to relapse, if the 70 degrees is not obtained studies have shown that the foot has a higher tendency to relapse. It is essential for this type of correction that the shoes be worn on both feet attached to the bar. This is key in maintaining the correction, both in dorsiflexion and outward rotation. If based on these descriptions you find that your son's foot does seem fully corrected there is a pdf file in the files section of this group's website that has tips and tricks for using the shoes. If you are not sure his foot is fully corrected you may wish to speak to your doctor about additional casting to achieve this or seek a second opinion. If you let us know who is treating your son, some other parents may be able to tell you of their experiences and/or direct you to another doctor in your area who may be more experienced with the Ponseti method. He is following the Ponseti method, and we really didn't have many problems with the casting. It may be that we need another cast or two. It just seems like he is in pain constantly when his foot is in that shoe. If Dr. Aronson is following the method, but just needs to do a couple more casts for your son this could be a good experience to see how he does in perfecting his technique. It would be wonderful for other parents in that area of the country, right now the closest really qualified Ponseti doctor is Dobbs in St. Louis. Her right foot never stayed in the brace. Due to issues with our other doctor, we're transfering to Children's Hospital in Boston next week. i sympathize with the non stop crying because my daughter was the same way. I wish you luck. Guest guest Posted October 27, 2005 Report Share Posted October 27, 2005 I want to chime in here. My son, , experienced the same problem. He originally was placed in the Markell/DBB for BL CF. He could not keep them on. At first I thought he was being tempermental but then we figured out he was fully corrected. We saw a doctor at our Children's Hosp. in Arkansas who uses the Ponseti Method. My doctor here said just keep trying the brace and we finally put our foot down after much crying from , all the crying I did, all the fussing my husband did and emailed Dr. Ponseti who responded to us and requested that we come to him for treatment. Even though our doctor told us Nichoals was " fully corrected " that was just abunch of lip service and was not fully corrected and therefore the shoes/bar would never work which then lead to a relaspe of both feet. also had some atypical features which also caused a problem. After two plus weeks in Iowa, recasting we are now in the Ponseti/ brace and shoes which has made our lives heaven. does not fight us or cry when we apply the shoes. We are all so happy now. I can enjoy his infancy. I finally feel as though CF is not running our lives anymore. Please research, ask questions and maybe consider emailing Dr. Ponseti himself. We can only speak for our children and my little man is one happy camper since we have had him fully corrected and in the new shoes. Please feel free to email me personally if you want to talk off boards. I am happy to share the experiences because there are plenty.
