ok, here are my 5 cents worth

I would write a letter to the Mayo, the board would be a good place to start, and to Dr. W., requesting a formal , ie: written response, to what excatly happened to the biopsy/ies. Send it registered receipt return or Fed Ex, something that shows they accepted delivery. I would also include a depiction of the evetns from your November 05 visit. In addition I would ask them what they intend to do to rectify this situation for you.
