No IV's, no starvation.....it's a simple thing. My husband held our son while the dr. did it in Iowa. s. tendon release surgery? Im SCARED! My daughter was born with her R foot, Club Foot. She is going in mext Tuesday for the tendon release (tendomy?). They told she may or may not have an IV. She could NOT eat breastmilk (or formula) 4hrs before surgery, and no clear liquids 2hrs before. SHE IS A MONTH OLD! HOW DO THEY EXPECT ME TO STARVE HER LIKE THAT!? They said the surgery itself would take about 20 minutes to 45 minutes. She will be put to sleep by gas. Her surgery isnt until 2:30pm. I COULD REALLY USE SOME SERIOUS ADVICE, SUPPORT, and others peoples stories of how their child did. How many of you had a child that had this done? Did they have an IV? Where did they put the IV? How do you keep them from screaming when they are hungry and you cant feed them? How long did it take? PLEASE, any advise is wonderful! Guest guest Posted August 24, 2005 Report Share Posted August 24, 2005 At Dr. P's clinic in Iowa, these young babies are given the tenotomy in his little " procedure " room, just a room where they do this thing. Like I said, my husband went in with our baby, I stayed in the hall with our older child. A numbing agent ws rubbed on his heel area, then a small knife dh described as being about teh size of a hyperdermic needle was inserted through the skin. He said the doc sort of gave the knife a quick little twist and he could see the tendon " snap " , suddenly the foot could move as the tendon had been released. The feet were carefull casted in the proper 70 degree / 10 degree position at that point, and we were sent home. I don't know your situation but I just don't see knocking a baby out for all this as necessary until they're much older and then it's a different situation. s. tendon release surgery? Im SCARED! I'm not going to lie, it was probably the most stressful time we ever had with her whole treatment. But in the end, we realized it was a LOT easier than we expected. It was mostly just in our heads! She too couldn't nurse for 4 hrs. before, so I set my alarm (we had to be at the hospital at 11:00) to make sure I woke up and nursed her nice and long just before the cut-off. Then we gave her a big bottle of water right before the 2 hour cut-off. That was her very first bottle ever, and she took it fine (from my husband). (I did add just a *touch* of sugar to it, that may have helped) The surgery is VERY short--we went down to get a cup of coffee and missed the doctor coming out to say it was all done. Maybe 20,25 mins total. The hardest part is the waiting. Ava was actually surprisingly calm about the not eating part. I would suggest maybe seeing if they can make your surgury a little earlier. At our hospital, they did the littlest ones earliest, and I think that helped. She did have an IV, which they do after they're asleep at our hospital, and she was put to sleep. Both of these things are really scary, but believe me, not as scary as you think right now. The day will fly by, and at the end you'll be absolutely amazed when you see how her foot looks. Just cuddle her lots, and try to stay (relatively) calm, because your baby can feel your emotions, and will take her cues from you. Try not to worry too much, and DON'T read all the warnings on all the forms they make you sign. They'll just make you worry more!! They HAVE to put everything possible down. They did not insert her IV until she was asleep. The hardest part for me was handing my baby to these strangers but she did great. She was pretty upset when she woke up and we had to ask for more pain medication for her but the staff in recovery was great and since she was my child and I knew her signs etc, they trusted my judgment that she needed more pain meds. We took her homw afterwards and she slept most of the day and all night but woke up the next day like nothing ever happened and didn't even require the tylenol anymore. They gave us a prescription for tylenol with codeine but we never had to open the bottle. In surgery, they also let her take her pacifier and her special blanket so that she had her comfort objects there with her. Like said, I would nurse her like crazy until your cutoff and then you could give water etc. She will be OK though. The IV will be put in to prevent dehydration which is a major concern with infants not eating. It will go by remarkably fast for everyone. If they let you nurse her in recovery, that will probably help a great deal with making her feel better and to be comforted. If you think she is in pain or other problems, do not be afraid to speak up. No one knows your daughter as well as you do so be insistent that they keep her comfortable. They told us no food or liquid after midnight, we had to be at the hospital at 6:00 and the procedure was at 7:00. It took longer to do the paperwork and wait for the doctor to be ready than it took for the tenotomy. I thought I would have a screaming baby, but he was really very calm.... Until it was over and the nurse tried to give him a bottle of sugar water. I think that was the worst part. He woke up scared and disoriented and they tried to give him a bottle (you have to understand that this child has never taken a bottle, he just won't). That made him really mad. They got him really worked up before they came and got me. All in all, the worrying was the worst part. We were done and in the car to drive home by 10:30. I did spent quite a bit of time in recovery calming him and getting him to sleep so he would sleep at least part of the drive. If you have other children, make sure you have a good babysitter with no other plans; just so you don't have to worry about them or hurry back. This is an option, though you may have to seek out another doctor to do it this way. Dr. Ponseti oversaw my daughter's tenotomy in the office at the U of Iowa, she was 5 months old at the time. Secondly, however, I do want to ask about the doctor that your daughter is seeing now. If I recall correctly from your post on the clubfoot board, you're seeing some doctor in Oregon. Are you confident that they're following the method correctly and that the foot is indeed corrected and ready for the tenotomy? I don't want to put any additional anxiety in your heart- but not knowing much about your doctor, I just want to make sure that you're confident that she's actually ready for the tenotomy and that the method has been followed properly thus far. Have you read the parents' checklist and the Global HELP booklet to see if you find any " red flags " about what you've been told about her treatment and the expectations for post-tenotomy bracing? Too many kids have had the tenotomy under the care of a doctor who wasn't doing the method right- and then they ended up having to have more castings and sometimes another tenotomy after switching to another doctor who knew what they were doing. I hope this isn't the case for you- but I just wanted to ask since I'm not familiar with Lilee's case other than what you've posted on the clubfoot board. They can have up to 8 casts before they perform the tenotomy. My son only had 5 casts but he really should have had another couple of weeks as he didn't tolerate the shoes so well until he had to have another week of casts then the shoes were ok.... where are you being treated? They really shouldn t be left without casts at this stage of the treatment. Is she having full length leg casts?? My son never kicked his casts off, I don't think he could have. It was just like any other visit, except they put the numbing agent on, left us with him for a bit for it to take effect, came back and within minutes it was over. The longest part of the entire process was waiting for the numbing stuff to take effect, and then maybe putting on the casts. And as I mentioned in my post on the other board, Aleksander didn't utter one peep during the tenotomy, and only cried when they put the casts back on. He's 9 months now, is crawling and pulling himself up, and has beautiful, perfect little feet. If you can, ask about using the topical numbing stuff instead of having your child put under. Especially at such a young age? Why do that when it isn't medically necessary? It does not sound like enough. Go with your gut. Call Dr. Ponesti about this Dr. and their methods. My son is 14 months. He was casted at 3 days old. He had 5 casts beore the tenotamy which his Dr. felt was a quick progression but was on track. We were told not to feed him before as to avoid him spitting up if he cried. They are woried about them choking. However, we were given the first appointment of the day, so he did not have to wait. We were told no liquids four hours before. The procedure was done in a casting room. He was numbed with a shot, we were then asked to stand outside of the room. There were two nurses tending to him and holding him while the procedure was done by Dr. King and his assistant. It took less than 15 minutes when we were called back in (only right clubfoot). He then had a full-leg thick cast for almost three weeks. His was then put in the DBB shoes, 23 hours a day. He started pulling himself up to a stand at 9 months and was put in the shoes only at night. He began walking 1 week after his first birthday. His foot looks great and he's starting to run in little spirts. Please contact someone to help you with this, i.e. Dr. Ponseti or his assistant Joyce. You can always reschedule if you decide to go with this Dr. but it doesn't sound right to me or anyone elses replies I have read. Good Luck, we will be thinking of you. Some surgeons hate operating on a crying baby, which is why they prefer the general. If your surgeon won't do it, then find one who will - I thought Ponseti did his by local.. Guest guest Posted August 25, 2005 Report Share Posted August 25, 2005 Dr. Dobbs does most of his patients tenotomies under general anesthesia because he works at Children's Hospital. I think it is a policy of the hospital that pretty much any procedure be done this way; the philosophy is that part of their reputation is about providing superior service and no pain for children. It was a little scary when I let myself think about my baby (6 weeks old) being put to sleep w/ Anesthesia. But that's what these people do - it's their specialty, Pediatric Anesthesiologists have LOTS of specialized training so they know how much anesthesia to give to a baby and when to stop giving them anesthesia so they wake up at just the right time and feel as little pain as possible (preferrably none). Sammy was a real champ though, he did not complain about the not eating thing -- he was really just paying attention to what was going on around him, and the whole thing was over so quickly, maybe 25 minutes from the time they took him to the time they came to get me. They let me nurse him in recovery and then he fell asleep. He dozed off and on for about an hour and nursed again and they sent us home. It was really much more of a big deal in my own mind before hand than what it turned out to be. Hope this helps! Mollie is three months old today and is in her second set of casts from Dr.Dobbs (3rd week though because he wasn't in this week). I'm not looking forward to it, but I can't wait to get done with all these casts so Mollie can have a real bath again!! He awnsered al my questions, and you could tell he was really concerned and cared. ) Lilee has has 6 casts. Her foot is at about a 45degree? angle. Dr. Ponseti said it shouls be between 60-70 BEFORE the tendontomy. He also said she should be in a cast, NO breaks! He said her DBB should be set at 70degrees, no if, ands , or buts. He asked is her heel is flat, and it is. He said this was due to poor molded casts. He asked if her foot is short, stubby, and fat. It is. He said this was called an A-typical CF. Anyone else have this? Whats the difference? Anyhow, Dr. Ponseti said that I need to see a different Dr. and get a second opinion cause this Dr. was not following the Ponseti method 100%, and it has to be! He referred me to Dr. Suffman at Shriners in Portland,Or. He also meantioned another in Seattle, Wash. He said if neither followed the method 100% to come and see him, himself. I would love for him to just do it, but we dont have the $$ to do that! Anyhow, I think we are going to cancell the tendonotomy for now,and wait to get into Shriners and see what they say? I am concerned of her losing alot/all th eprogress we've already made. Any suggestions on what I can do to keep it as it is? Like I said, my husband went in with our baby, I stayed in the hall with our older child. A numbing agent ws rubbed on his heel area, then a small knife dh described as being about teh size of a hyperdermic needle was inserted through the skin. He said the doc sort of gave the knife a quick little twist and he could see the tendon " snap " , suddenly the foot could move as the tendon had been released. The feet were carefull casted in the proper 70 degree / 10 degree position at that point, and we were sent home.
