Is this something that Ponseti may have done? or is it outside the Ponseti technique? Henry has just had an ATTT, about 6 weeks ago, but is still having a little pain, and some of the recent posts have shown how careful we all need to be about taking the docs word for granted, especially those who say they're Ponseti but they're NOT!! Guest guest Posted July 30, 2005 Report Share Posted July 30, 2005 Hi Dan Our doctor was a little vague about it - what he said was that he would re-attach the tendon, and " depending on how tight things were in there " , which he wouldn't know until he was able to see inside, maybe do a capsule release. This apparently increases dorsiflexion considerably. You will knwo what its like in a consultation - despite the fact that we'd written most of our questions down, there were always a couple after you'd come away, and I've always been just a little worried that maybe a " capsule release " is a non-Ponseti trick. If I don't get any more ideas from the group, I suppose I could always e-mail Ponseti himself. As for increasing dorsiflexion, Dr. Ponseti didn't recommend doing anything for Emma's even though she only had about 5° on her left foot. However, the operation he described for increasing dorsiflexion involved a heel cord lengthening procedure which is significantly more invasive than a simple tenotomy. The biggest signal that there's a problem is that Jake can't flex his foot upwards very far, as he walks. There is also a lump on the top of his foot where the tendon was reattached, and Dr Dietz says that this happens when the tendon pulls out a little. Guest guest Posted August 2, 2005 Report Share Posted August 2, 2005 - Actually Jake does need to have his ATTT redone; we have it scheduled for Oct 18th. The tendon just isn't lifting his foot well enough and Dr Dietz doesn't want to wait too long to fix it. If we wait too long (ie until Jake has major problems) then we run the risk of his foot having stiffened up. Dr Dietz says that Jake's foot is still quite flexible at this point so he won't need to do any surgery in the joint areas; it's the joint surgeries that cause ppl to have foot pain as adults. In addition to a 2nd ATTT surgery, Jake will also have another achilles tenotomy as well as some work done on his arch support. For some reason Jake's arch is very high causing his left foot to be a size and a half smaller than his right. It is also kind of hard to put shoes on that foot- we have to tie them really loose. Dr Dietz said the arch surgery will be similar to the achilles release. Hope this all makes sense.
