My insurance approved quickly and I was moved up quickly. I did however, have my stuff (teacher---a months worth of lesson plans and materials) in order. I also had it figured out how they would cover my classes (a suggestion that they took). I also never gave my immediate boss (department chair) the official note. I just sent that to the personnell department. You gotta learn to work the system. Remember that they work for YOU, not the other way around. It is YOUR body, YOUR health, YOUR life. Part of the reason I don't get any crap from Kaiser is that I don't TAKE any crap from Kaiser, or anyone else! I will add that I used to work in an office that provided medical employee benefits using several insurance companies, and Kaiser was the best overall. Doesn't mean it's perfect though.
